Everyone in our center is devoted to your care, happiness and well-being. We will be working with you to establish goals and expectations for your stay. Below are just a few members of your healthcare team.

Our Licensed and Certified Nursing Staff

Our licensed nursing staff will participate in designing and implementing your personalized treatment plan. Our Certified Nursing Assistants will help you, around the clock, with your personal needs

Social Services

Social Service staff will provide ongoing psychosocial support to patients, residents, and their representatives. A social services specialist will be your information resource supporting you and your family throughout the stay and answer any questions you may have. Our social service specialists will facilitate discharge planning when appropriate, including the coordination of home and community based services, equipment needs and other community resources. This individual will work with the interprofessional team to ensure your transition to home is smooth and to confirm that any planned support services are in place.

Our Rehab Therapy Team

Your rehabilitation therapist is focused on delivering all levels of physical, occupational, speech and respiratory therapy to meet your needs. Our nursing staff will work closely with your therapist(s) to promote your independence and safety with your activities of daily living.

Recreation Specialist

The recreation specialist ensures that you have an opportunity to participate in meaningful programs and activities that enhance your vitality, sense of well-being and contribute to attaining and maintaining your highest practicable quality of life. Depending upon your type and level of treatment the recreation specialist will work with you in a one-to-one, small group or large group program or through self-directed activities in an attempt to reach the goals set forth in your treatment plan.
